Insurance

Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

• What is my health insurance coverage and benefits?
• What is my deductible and has it been met?
• Do I have a co-pay? If so, how much is it?
• What is my wellness visit coverage?

Out-Of-Network provider

I am an Out-Of-Network provider. You will need to be responsible to provide payment at the time of your appointment.  For insurance plans, I provide patients with an insurance suberbill that you can submit to your insurance carrier for reimbursement consideration. Check with your insurance as many plans reimburse a significant portion of the cost. 

Payment

Cash, check, and all major credit cards accepted for payment. Payments can be made at any time by going to Make a Payment.  

Cancellation Policy

If you do not show up for your scheduled appointment and you have not notified us at least 24 hours in advance, may be subject to cancellation fees.

Concierge Medicine

We strive to provide Optimum quality care for any and all of our patient’s needs.  We can arrange Concierge Medicine and any in-home or hospital visits for patients who prefer the privacy of their own homes or offices. Separate fees to apply. Please contact us for any needs you may have.
